As the Senior Living Lead Therapist, you will be responsible for the day-to-day clinical care of patients on HH and Outpatient Services. Process orders and attend clinical meetings within the facility. You’ll also be responsible for operationalizing all components of the SL plan in their facility/facilities. Reports to SLD for all operational, growth, quality and compliance matters in their facility/facilities. Primary Responsibilities: Documents all Outpatient and Home Health visits per company standards and initiates care promptly for Start of Care cases Schedules therapy sessions appropriately and identifies patient and family education needs Delivers safe and effective treatments using approved techniques and protocols, adhering to medical precautions and equipment safety Coordinates interdisciplinary goals, secures physician orders, and plans care based on financial and accessibility considerations Participates in team meetings, performance improvement, staff training, student supervision, and ensures documentation meets company standards You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Recent graduates who have applied for the registration exam may be considered for provisional appointment Current Physical Therapist, Physical Therapy Assistant, Occupational Therapist, Occupational Therapy Assistant, Speech Therapist, or Licensed Practical Nurse licensure in the state of practice Current CPR certification Membership or certification in relative national associations Driver’s License and access to reliable transportation Preferred Qualification: Management experience Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $71,200 to $127,200 annually based on full-time employment. We comply with all minimum wage laws as applicable.
