  • Salary: $112,396 - $134,500 (Pro-Rata) + Super + Salary Packaging
Responsibilities

ABOUT THE ROLE:

  • Max-term full-time role delivering high quality, evidence-based mental health care and support to clients of the Wesley Mission SHS Team who live with mental illness and are experiencing or at risk of experiencing homelessness.
  • Deliver comprehensive assessments, clinical intervention, care coordination, and referral on outreach alongside Wesley Mission staff.
  • Collaborate with clients, multidisciplinary team, Wesley Mission, and stakeholders to address cultural, holistic health, and support needs using a strengths-based, harm reduction approach.
  • Build strong partnerships and coordinate care with service providers and communities to strengthen client support networks.
  • Assist clients in accessing mainstream and specialist healthcare, support services, and stable accommodation to improve health and wellbeing outcomes.
  • Lead staff education, contribute to clinical reviews, and drive quality improvement across services.
