As part of our Specialty Pharmacy team, you are responsible for providing clinical and operational support for the ambulatory clinics in the Specialty Pharmacy service line. This position will have duties related to clinical patient video visits. You are also responsible for some inpatient job responsibilities such as inpatient order set verification. We are looking for an employee who is eager to learn new skills and wants to grow in their professional career. Your exceptional skills You are knowledgeable in benefits investigation, financial support, clinical monitoring, education, and dispensing a variety of sub-specialty services. You have the ability to create and maintain clinical pharmacy pathways for initiation and monitoring of specialty medication therapy for self-administered medications. This includes step therapy requirements, patient education, and monitoring of applicable laboratory, pharmacokinetic, and pharmacogenomics results You are also knowledgeable in treating patients that require medication therapy for conditions such as; Hepatitis C, Cancer, Cystic Fibrosis, HIV You have knowledge of inpatient order verification You are available in the evenings and weekends as needed Your exceptional qualifications You have earned a Bachelor's degree from an accredited pharmacy program. Doctorate degree preferred You currently hold an active and valid Indiana Pharmacist license You have four (4) to five (5) years of experience in a specialty, ambulatory, hospital based, or a closed-door long term care pharmacy Your life at Community You work hard to provide our patients with the exceptional care, and you deserve benefits to match.
