About the role

• The purpose of this position within the Complex Chronic Disease Team (CCDT) is to apply high level physiotherapy skills and knowledge in the assessment and treatment of patients/clients with various and complex chronic conditions in the local geographical population in order to promote, maintain and improve their health, functional ability and quality of life.
• The role will be responsible for establishing referral pathways for pulmonary and cardiac patients who are suitable and eligible for virtual pulmonary and cardiac telerehabilitation in Central West to establish and run an efficient and evidence based virtual pulmonary and cardiac rehabilitation service.
• This role will support the implementation transition to business as usual of the Metro North Central West Respiratory Partnership - Virtual Pulmonary and Cardiac Rehabilitation
