A Senior Facilitator will function as a peer leader and is an experienced Practice Facilitator who is at the expert level of clinical workflow knowledge in the physician practices and has an expert skill set as a facilitator. The role will function to onboard and train new Practice Facilitators, evaluating and ensuring their competency. This role will also mentor new Primary care managers to orient them to the work and goals of primary care transformation. The Senior Practice Facilitator will function as a subject matter expert for Medical Home Model workflows and will be a subject matter expert for both MIPS and HEDIS Measure specifications, dashboarding, action planning tactics. The Senior Facilitator will have a broad scope in mentoring the novice facilitation team in Infection Prevention, Practice Standard operating procedures, and Patient Centered Medical Home scope and standards. The Senior Facilitator will coach the facilitation team on Team Steps for Physician Practices, Motivational Interviewing, relationship building and team building. Responsibilities A day in the life of a Senior Facilitator at Hackensack Meridian Health includes: Subject matter expert with Quality Measure Management (Hedis and MIPS), Subject matter expert for reporting and dashboarding for quality measures, action planning with the physician practices, Subject matter expert HMHMG Panel Management activities and Annual Wellness workflows, Subject matter expert Patient Centered Medical Home accreditation, Must display core traits and behaviors consistent with HMHN 5 Culture Creative, Compassionate, Collaborative, Courageous, and Connected. Work independently to write curriculum tied to PCMH and Quality Workflows. The Senior Facilitator will: Act as mentor for novice facilitators and new Primary Care practice managers as related to transformation and quality. Will continue to hold practice assignments and provide training to assigned practices on the contents of specific transformational and quality improvements projects. Will maintain a practice assignment in addition to added responsibilities of mentoring novice team members. Qualifications Education, Knowledge, Skills and Abilities Required: 2 years of experience as a practice facilitator on the Ambulatory Quality and Patient safety team. Working towards PCMH CCE, or working towards Bachelors degree, to be obtained within two years of hire or transfer into the position. Need to drive and have daily access to a car. Travel required between assigned practices is required. May need to cover any practice within the Medical Group depending on organizational need. Education, Knowledge, Skills and Abilities Preferred: Bachelor's degree in any Health Care related field of study. Master's degree in any Health Care related field of study. NCQA PCMH Content Expert Certification. Experience as an LPN (Licensed Practical Nurse) , CMA (Certified Medical Assistant) orPractice Manager in a physician practice. Proficient use of google sheets, forms, and slides. Ability to present quality outcomes using effective communication skills. Skilled communication is essential for the PF in order to maintain effective and sensitive relationships with the members of the care team. Requires a comfort level using EMR and clinical data, clinical reporting and analytics tools such as clinical databases. Excellent clinical skills, teaching skills, interpersonal skills, analytically minded, able to learn systems quickly. Licenses and Certifications Preferred: LPN or CMA.
