    Mission Australia is seeking a dedicated Senior Recovery Case Worker to support clients and colleagues of the Community Living Supports (CLS) program in delivering person-centred, culturally responsive care.
    This role involves working closely with individuals with complex needs, guiding them towards independence through tailored, collaborative case management plans and ongoing support. You will play a key role in supporting clients transition out of our service, empowering them to self-manage and access the resources they need.

Your key responsibilities will be to:

  • Provide person-centred case management in line with Mission Australia’s National Case Management Approach, with complex clients and review progression against case plan.
  • Provide case management sessions with clients that are sensitive and responsive to, the cultural strengths and needs of each client.
  • Assist clients while at the service through support intervention and referral to suitable services.
  • Actively contribute to continuous quality improvement and the development, implementation and review of relevant service delivery practices.
  • Assist clients in the process of transition out of the service into independence or other services.
  • Build and maintain positive internal and external relationships, with community, government non-government services, Indigenous organisations, clients, and their family/caregivers etc.
  • Undertake administrative tasks including statistics, reports, letters, plans and data entry in relevant client management system, etc.

Requirements for Success

  • Hold a relevant degree or associate diploma with relevant experience is highly desirable
  • Experience applying case management approach to clients.
  • Demonstrated ability to manage a caseload of clients with complex needs.
  • Demonstrated professional approach to the role including maintaining professional boundaries.
  • Ability to provide specialised or complex therapeutic interventions and manage a caseload of clients who may have complex needs and multiple agency involvement.
  • A sound understanding of recovery oriented practice and a proven ability to use this to positively support others on their recovery journey.
  • A positive and person-centred approach with a strong guiding belief about everyone’s capacity to self-right and grow within and beyond their current circumstances.
