Programs include early childhood education, youth mentorship and leadership development, employment services, career readiness training, housing and financial stability, senior citizen services, affordable housing and neighborhood development, and arts and cultural events. Our work expands beyond the Fruitvale district and now reaches across Oakland and into Contra Costa County. The Unity Council employs a diverse workforce of more than 300 people who reflect the linguistic, cultural, and ethnic identity of the communities we serve. Department Summary The Community Programs Department offers integrated services to revitalize disinvested communities and remove barriers to equitable economic, career, housing and wellness outcomes for youth, adults, seniors and small-business owners. We serve Oakland and the East Bay through two core service hubs: our Career & Resource Center, which provides workforce development, career pathways and human services referrals; and our Business Assistance Center, which supports small-business development and access to capital. Beyond these hubs, we offer achievement and wellness programs for youth, coaching in employment and financial support, stewardship of low-income properties, and neighborhood revitalization through events, civic engagement, arts and culture, and urban planning. All services are multi-lingual and culturally rooted, designed with deep community accountability and available to Oakland and Alameda County residents based on funding. Through this integrated approach, we ensure all residents and businesses can shape urban development and benefit equitably from economic growth. Position Summary The Senior Services Coordinator works closely with older adults to help them stay healthy, connected, and independent. This position builds trusting relationships with older adults and their families, especially those who are frail, socially isolated, have limited English proficiency (LEP), or monolingual, by assessing their needs and providing engagement, service coordination, referrals, and connections to resources and benefits that promote safety, independence, wellness, and housing stability. Working collaboratively with community partners, the Senior Services Coordinator helps older adults access vital services that support aging in place with love, care, and dignity. Essential Duties and Responsibilities The duties and responsibilities include, but are not limited to the following: Senior Support & Service Coordination * Provide general assistance, advocacy, and referrals related to supportive and social services for older adults. * Conduct basic needs assessments to identify risks related to health, safety, isolation, and housing stability. * Assist participants with benefits navigation, including Medicare, Medi-Cal, Social Security, SSI, and IHSS (no eligibility determinations). * Support residents in setting up personal safety and stability tools, such as medical alert systems (e.g., Life Alert), automatic bill payments, and direct deposit. * Coordinate referrals for in-home supportive services, transportation, meal delivery, healthcare access, and mobility resources. * Provide culturally responsive case management, including assessment, service planning, follow-up, and coordination of community resources to address participants’ social, health, and wellness needs. * Maintain collaborative relationships with senior service providers, healthcare-related organizations and community partners. * Coordinate visiting service providers and on-site services as appropriate. * Accurately document participants interactions, referrals, and outcomes. * Maintain organized resident service files and assist with required reports. * Attend required meetings, trainings, and professional development activities. * Perform other duties as assigned to support the operations and programs of the Senior Center. Community Programming & Engagement * Assist with planning and coordinating on-site wellness, educational, and social activities tailored to seniors. * Support food access programs, including coordination of food distribution and outreach. * Promote resident participation and reduce social isolation through community-building activities. * Conduct outreach by attending community events and resource fairs and other engagement opportunities to promote the Senior Center, connect with older adults, and assist with participants enrollment. Safety, Crisis Response & Mandated Reporting * Respond appropriately to crisis situations using de-escalation techniques and established protocols. * Identify and report suspected abuse, neglect, or self-neglect to Adult Protective Services and internal supervisors, as required by law. * Promptly report incidents, safety concerns, or observed risks. Qualifications * Education: High School Diploma or GED required; Associate’s or Bachelor’s in Social Work, Human Services, Psychology, Gerontology, or related field preferred. * Experience: 1–2 years of experience in resident services, social services, or community-based work. Experience working with seniors or older adults preferred. * Knowledge: Familiarity with senior service systems (Medicare, Medi-Cal, Social Security, IHSS). * Skills: Strong communication, problem-solving, and organizational abilities; able to work independently and as part of a team. * Technical: Proficient in Microsoft Office and data entry systems. * Language: Bilingual English/Spanish required. * Other: Flexible schedule, including occasional evenings/weekends. * Values: Commitment to The Unity Council’s mission of equity and inclusion.
