Responsibilities
  • Provide exemplary social work service and leadership in the Inpatient Rehab Unit, including assessment, intervention, and discharge planning.
  • Foster positive team and workplace culture as a Senior Social Worker and Lead in the multidisciplinary rehab team.
  • Utilise specialised skills and knowledge in relation to relevant hospital and community providers, communicating between patients and caregivers.
  • Demonstrate excellent communication strategies with patients, relatives, staff and relevant services in order to provide appropriate services, information and leadership.
  • Actively lead and participate in Allied Health & Rehab quality improvement initiatives and research.
