We are excited to announce a new opportunity for a Senior Social Worker to join our newly established Medway’s Intensive Support Team (MIST) within Adult Social Care. The successful candidate will play a pivotal role in supervising, leading and mentoring a small team of Social Workers and a Social Care Officer, ensuring the delivery of high-quality support to individuals with diverse needs, and in addition may be required to hold complex cases
The role will be responsible for complex case management, providing expert guidance on assessments and care planning while advocating for clients’ rights and wellbeing. In addition, you will be expected to oversee the implementation of safeguarding policies and statutory requirements, with the expectation to collaborate closely with multi-disciplinary teams and external agencies to coordinate comprehensive care solutions. This role is one which will lead by example, modelling good practice and utilising oneself as a tool to engage directly with individuals, often in crisis situations,
This role presents a unique chance to make a significant impact on the lives of individuals facing multi-faceted complexities, providing hands-on support and interventions within the community, including but not exclusive to acute settings, individuals’ homes, and numerous other settings. The successful applicant will be required to have experience within Safeguarding, Mental Health, Learning Disabilities, Substance and Alcohol Misuse, Homelessness and outreach work.
As a Senior Social Worker in this dynamic team, strong communication skills are essential, as you will be required to build and maintain relationships with individuals accessing support, their families, and partner agencies, including Housing, Police, Probation Services, Mental Health Professionals, and Addiction Support organizations.
Knowledge of the Medway area is crucial, as you will be navigating local resources and voluntary services to enhance the support available to individuals. Whilst in Medway we support agile working, given the nature of this work, it will predominantly require a presence in both office and community settings, emphasizing direct engagement with clients to effectively address their needs.
You will need to apply a strength-based approach in your practice, developing comprehensive care plans through our three-conversation framework of assessment. A solid understanding of safeguarding and the role of an Enquiry Officer, the Mental Health Act, and the Mental Capacity Act is fundamental to ensure the safety and wellbeing of those you support.
