Job Description Leads, guides, and coordinates staff, students, and support staff in the performance of duties. Provides social work services to patients and/or their families throughout the continuum of care. Performs all functions of Social Worker. Job Responsibility   * Assists in the formulation and development of departmental and interdepartmental programs, policies and procedures.  * Contributes to the assessment of social health needs in the facility and community; participates in the development of effective health and welfare services/resources.  * Collaborates with other staff members to identify unmet psychosocial needs of patients.  * Participates in appropriate research, staff development programs, and community liaison activities.  * Contributes to educational activities including presentation, training of graduate students, attending/championing workshops and seminars.  * May participate in hospital committees to discuss social work practice and procedures.  * Performs related duties as required.
