Are you a passionate, and caring nurse with a least two years’ experience in community looking to progress and develop your clinical and leadership skills in a dynamic community nursing team?
A vacancy has arisen within the Unplanned Care Team, Triage Hub, office- based at Moorlands View, Lynfield Mount Hospital, which has an innovative approach to nursing care. It involves decision making and quick responses to patients in their home to assess, plan, manage and evaluate the presenting issue, prevent unnecessary hospital admissions, and help to facilitate early hospital discharges. You will support the clinical band-7 with the daily leadership and management of the team; and liaise with the wider multi-disciplinary team.
The post holder will be responsible for the assessment of health needs. The current hours of work are Monday to Friday 8am to 4pm, however these will alter to a seven- days-per-week service, 08:00 – 20:00 as the service expands.

The Senior Triage Nurse will provide triage assessment, clinical support, assistance and professional expertise to patients, carers and health and social care professionals accessing the Community Nursing Service.
The senior staff nurse role supports the Triage Hub team leader with the daily service coordination and delivery; and is pivotal in the communication within the team. The senior Triage nurse is also responsible for providing expert clinical support and guidance to other members of the Triage Team, and those accessing the service. You will be expected to deputise in the absence of the team leader.
