This role offers the opportunity to lead by example, support our clients, and assist our team in delivering exceptional care.

• Assist clients with their daily routines, offering companionship and ensuring their safety and comfort.
• Conduct client assessments and reviews, including care planning and risk assessments.
• Manning the on-call phone on rotation with the office team, providing timely support.
• Navigate and work effectively on our Birdie support system (computer skills required).
• Build and maintain strong working relationships with Social Workers, GPs, staff team, clients, and their families.
• Act as a role model for the care team, promoting best practices and teamwork.
