REQUIREMENTS

  • Ontario Secondary School Diploma or equivalent and a minimum 2 years’ experience in the Human Services field or Post-Secondary School Diploma in a related field and a minimum of 1 year experience;
  • Vulnerable Sector Check (6-months current)
  • Valid Class “G” Ontario Driver’s License (agency vehicle will be available while providing supports)
  • 3-year driver’s abstract
  • Proof of $2million third party liability insurance

Responsibilities

DUTIES:

  • Monitor the adherence to all Quality Assurance Measure (QAM) requirements;
  • Act as the primary conduit between the work location and Supervisor;
  • Support people in their daily routines, as required in the Agency’s Vision, Mission and Service and Guiding Principles, and other Agency philosophical statements;
  • Develop and implement Individual Support Plans, as required;
  • Maintain effective, open communication and cooperation with people receiving service, co-workers, families, external services and Supervisor;
  • Act as the primary contact/liaison as required/directed for people supported on “caseload”;
  • Provide flexible, quality supports to assist people to discover inclusive, community options;
  • Assist people to be as independent as possible to develop natural supports;
  • Provide quality support consistent with all Agency Policies and Procedures, including the Occupational Health & Safety Act (Section 28) and Medication Policy and Procedures Manual;
  • Report for duty in emergency situations, if available.
