Hours for this position are primarily day shift. Must be able to work some evenings and weekends according to a rotating schedule. Eagleville Hospital, located in Montgomery County, Pennsylvania, is a private non-profit organization dedicated to the treatment, education, and prevention of substance abuse, co occurring disorders, and acute psychiatric disorders. The hospital serves as a regional resource for the treatment and prevention of alcohol and drug abuse, as well as a national resource for training and research for addiction and related conditions. Responsibilities include: Assists coordinator in developing and facilitating daily case conference Completes a daily report outlining all patient activities Manages the daily activities and schedule of treatment team staff. Manages counselor assignments and discharge dates Prepares Personal Histories, Bio-psychosocial Assessments, Comprehensive Treatment Plans, Treatment Plan Reviews and Revisions, and ASAM Conducts group counseling, individual counseling, family sessions, psycho educational and specialty groups with clients as assigned and documents appropriately and within hospital timeframes Assists clients in maintaining contact with external systems when necessary including: Probation or Parole Officer, DPA, DHS, Employers, Funding Sources, and other case management as required Maintains treatment planning and patient progress in the documentation tracking system Conducts and participates in case conference with patient and treatment team Eagleville Hospital is an Equal Opportunity Employer Qualifications Requirements: The minimum qualification is a Master's Degree in a health or human services field Two years of clinical experience in a health or human service agency, preferably in a setting that serves individuals with co-occurring mental health and substance abuse disorders is required.
