JOB SUMMARY:

Responsible for collaborating with the multidisciplinary healthcare team and facilitating activities/services that drive improvements in sepsis outcomes by combining the roles of clinician, leader, liaison, educator, and quality improvement specialist. The Sepsis Coordinator is responsible for ensuring the provision of reliable, high value, evidence-based and coordinated care for adult sepsis patients in the ED and inpatient settings. The Sepsis Coordinator facilitate evidence-based practice across all disciplines in accordance with current sepsis guidelines while working directly with providers and multidisciplinary teammates as necessary to facilitate the care for sepsis patients that is consistent with National guidelines and the Organization’s Sepsis Program goals.

Responsibilities
  • Conducts periodic clinical shift support as required to improve performance outcomes.
  • Makes regular rounds on sepsis patients and routinely attends Interdisciplinary Rounds to evaluate appropriateness of orders, treatment plans, nursing intervention, physician documentation and compliance with the Sepsis bundle to assure that all patients receive comprehensive and timely treatment interventions that are consistent best practice guidelines.
  • Facilitates close multidisciplinary partnerships between the ED, ICU and other clinical departments.
  • Collaborates with the multidisciplinary care providers to identify on-going care concerns related to sepsis care and works with them to develop strategies for improvement.
  • Facilitates the design, implementation and evaluation of the Multidisciplinary Sepsis Program Charter and annual PI Plan to address all applicable regulatory, accreditation, certification and/or organizational requirements.
  • Facilitates the defining outcome and process measures of the Sepsis program for the severe sepsis and septic shock population such as: Improvement in Compliance with the Sepsis Care Bundle; Reduction in LOS; Reduction in the mortality rate; Reduction in sepsis related readmissions
  • Leads the facilitation of the multidisciplinary Sepsis team activities such as meeting agendas, scheduling of meetings, maintaining minutes and follow-up on assigned tasks.
  • Facilitates the development and periodic review and revision of Sepsis-related forms, including orders sets, protocols, checklists, flowcharts, policies, and procedures.
  • Works with the Electronic Medical Record support teams (Clinical Application Analysts and Clinical IS), Quality Management Nurses, Clinical Documentation Specialist, and Health Information Coders to improve/enhance documentation regarding Sepsis for nursing and physicians.
  • Participates in hospital community outreach program by providing sepsis awareness and early intervention education to the public.
  • Facilitate the development and delivery of Sepsis related education for healthcare providers and teammates across the continuum of care.
  • Completes periodic real-time and retrospective review of the sepsis patient’s EMR to ensure optimal sepsis care. Uses the findings of these reviews to communicate opportunities and improvement strategies to the frontline care team regarding patient sepsis care.
  • Facilitates the design and use of reports to identify potential sepsis patients and uses the reports to monitor compliance with the established sepsis treatment protocols and guidelines.
  • Facilitates the review 30-day readmission episodes for sepsis patients, in-hospital mortality, and 30-day mortality using a standardized review tool or protocol.
  • Remain abreast of all appropriate federal, state, and local regulations and guidelines including hospital policies and protocol changes
  • Works closely with the Quality Management to understand regulatory reporting requirements, review and consolidate quality related data for compliance with evidenced based metrics to prepare and present data to the appropriate Organizational and Medical Staff committee/team meetings.
