JOB OVERVIEW

A.I.M. Wellness is seeking aService Coordinator to provide person-centered Targeted Case Management. The Service Coordinator will help clients access services, reduce barriers, and make progress on individualized goals. This role requires a compassionate, organized professional who can connect clients with resources, support care coordination, and uphold compliance standards.

REQUIREMENTS

  • Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
  • Familiarity with community resources and Medicaid/managed care processes (authorizations, referrals, documentation standards).
  • Strong organizational, communication, and advocacy skills.
  • Ability to work effectively in fast-paced environments and diverse community settings.
  • Experience: Case management, social services, or behavioral health experience preferred.
    Job Types: Full-time, Contract
    Pay: $35,000.00 - $65,000.00 per year

Responsibilities
  • Conduct strengths-based needs assessments and develop individualized service plans; review and update plans as needed.
  • Coordinate referrals and linkages to community resources (e.g., medical, psychiatric, MAT, housing, transportation, employment/education, food, childcare).
  • Schedule and track client appointments; provide reminders, follow-ups, and re-engagement outreach after missed visits.
  • Monitor progress toward goals through regular contact with clients, family members, and home visits; document outcomes and adjust plans accordingly.
  • Collaborate with treatment providers, community partners, and organizations; assist clients in navigating systems of care.
  • Maintain accurate, timely case notes, service plans, ROIs, and quality metrics in the EHR.
  • Educate and advocate for clients regarding benefits, services, and resources.
  • Identify and address social determinants of health, including transportation, housing, and food security.
  • Provide non-clinical crisis support and safety planning within scope; connect clients with clinical staff or emergency services when appropriate.
  • Maintain a current resource directory and support program improvement efforts.
  • Ensure compliance with HIPAA, confidentiality, and documentation standards.
  • Demonstrate reliability, productivity, and professionalism in all client and team interactions.
