What will a day in the life of a Service Coordinator entail?

  • Participating in patient support planning at multi-disciplinary meetings in community venues and acute hospitals in Portsmouth.
  • Promoting the service and developing relationships with health and care partners to generate referrals through outreach and engagement.
  • Triaging referrals according to local service criteria and allocating cases for support, such as urgent welfare checks, transport, or food provision, within the required timeframe.
  • Ensuring that the impact of our service for service users is recorded through appropriate records (such as BRM), completing reports and paperwork as required.
  • Holding line management responsibility for the support workers, casual support workers and volunteers.Being a contingency for other staff during periods of absence.
