GENERAL POSITION DESCRIPTION

This position is responsible for the planning, execution, and review of services provided to the clients of HCEB. They handle complaints and questions of clients and reports to their immediate supervisor.

QUALIFICATIONS

  • Associate degree required/Bachelor’s degree preferred
  • History of progressively responsible related work experience, preferably in a non-profit organization, minimum of 2 years
  • Experience supporting homeless adults with disabilities, minimum of 2 years
  • Experience working as part of a dynamic, multi-disciplinary team
  • Strong record-keeping skills
  • Able to pass a fingerprint background check
  • Excellent PC skills, including word-processing and spreadsheets in a Windows-type environment
  • Knowledge of Clarity/HMIS/Coordinated entry assessments information
  • Ability to work in outdoor settings/various weather conditions
  • Results-oriented and able to meet deadlines
  • Excellent interpersonal and customer service skills
  • Flexible and self-motivated with an ability to coordinate and prioritize workload
  • Interest in growing with the agency, progressively assuming more responsibility
  • High standard of excellence and quality
  • Sensitivity and respect towards the individuals we serve
  • Driver License

Responsibilities

DUTIES AND RESPONSIBILITIES

  • Assist participants with the housing application process, coordinate health and housing services, and provide tenancy sustaining services
  • Initiate and maintain positive working relationships with local private and nonprofit landlords
  • Maintain accurate program participant files, current housing inventory, and provide weekly program participant updates
  • Maintain regular contact with other tenant support staff and address any issues arising from those conversations, in conjunction with property management staff
  • Perform housing outreach and administrative tasks, and participate in trainings and meetings
  • Coordinate with the finance department to track program participant’s Housing Financial Assistance accounts
  • Support residents who become delinquent in rent payments or receive other lease warnings from property management; support residents and their support staff in submitting maintenance work orders to property management
  • Participate in regular check-in property management staff to review resident red flags
  • Community-building activities
  • Accompany clients to appointments for benefits, employment, housing searches
  • Maintain professionalism, discretion, and independent judgment with clients
  • Ensure participant’s document-readiness
  • Encode service notes and housing related expenses into HMIS database
  • Support participants in lease ups; Request for flex funds to pay security deposit and first month’s rent and other housing related fees
  • Assist participants in applying for rental/ move-in subsidies
  • Facilitate regular community meetings at each property
  • Perform other duties as assigned
