Description:

OVERALL JOB RESPONSIBILITY

This role seeks to plan and develop the post-18 services so that PwDs can access appropriate and relevant care and residential services to meet their varied needs. This includes looking into the development of post-18 residential services to support PwDs and their families to live at home and/or in the community. This role should also review the institutional residential options, such as transitional care for IMH patients.

QUALIFICATIONS, KNOWLEDGE AND EXPERIENCE

  • At least 6 to 8 years of relevant experience in service management work or have run new programmes /pilots.
  • Good degree in relevant disciplines
  • Strong stakeholder engagement skills
  • Strong team player
  • Self-driven, resourceful and able to multi-task
  • Meticulous and diligent

Responsibilities:

  • Support the AD (Service Development) and DD (Service Development) on the development of post-18 services for PwDs, in particular the Enabled Living Programme (ELP) and the wraparound services to enable PwDs and their families to live at home and/or in the community.
  • Duties include the following:
  • Develop and refine service model and the necessary wraparound services for the ELP pilot to meet its objectives of supporting PwDs and their families to live at home and/or in the community;
  • Organise and facilitate consultation sessions to engage PwDs and caregivers on the ELP service model and wraparound services;
  • Develop the necessary assessment tools and referral workflow for the ELP;
  • Engage stakeholders including ELP operator(s), grassroots and other community agencies to ensure wraparound support for PwD residents;
  • Review community-based and home-based support services for PwDs with healthcare needs or behavioural challenges; and
  • Execute any other strategies related to operations and service delivery in support of SG Enable’s objectives.
