Description:
This exciting leadership role of a ‘Service Manager’ oversees the Community Team for People with Learning Disabilities (CTPLD), encompassing both Child and Adult and Adult Short Breaks (1 Willow Close), NHFTs specialist 6 bed respite bungalow for adults with severe and profound learning disabilities with complex health needs.
The post holder will utilise their clinical experience, knowledge of national and local drivers and compassionate leadership skills to lead the multidisciplinary teams (MDT) within Adult and Children’s CTPLD. The CTPLD consists of Learning Disability Nurses, Physiotherapists, Occupational Therapists, Speech and Language Therapists, Healthcare Assistants, Clinical Psychologists and Psychiatry. The post holder will provide operational management, line management, quality assurance and compliance with regulatory standards for the CTPLD and Adult Short Breaks. This role sits within the Senior Leadership Team for Learning Disabilities within the Mental Health and Learning Disability directorate, with opportunities to develop senior operational and strategic leadership skills.
This will contribute to the delivery of the safe and wellbeing agenda for community based care and treatment, hospital avoidance, reducing health inequalities ensuring the delivery of NHS plan.
The post holder will work alongside the Transforming Care Clinical Service Manager and across the Integrated Care System (ICS) partners in ensuring that people with learning disability and/or autism are supported to live a fulfilling and independent life in the community with the right support and close to home.
