POSITION SUMMARY

Reporting to the Program Director, the Service Planning/Admissions/Discharge Facilitator arranges, organized and facilitates Service planning for Individuals and their supports. The Service Planning/Admissions/Discharge Facilitator also facilitate the admissions and discharge processes for individuals entering or leaving services.
The Service Planning/Admissions/Discharge Facilitator facilitates Person Centered Planning or Personal Planning with the Individual and their Natural Supports upon request. The Service Planning/Admissions/Discharge Facilitator ensures that the service planning, admission and discharge processes are of high quality, adhere to the philosophy and policy standards of the association and promote a natural and normal life for each Individual receiving services.
1. Specific Duties and Responsibilities

a. Individual Support

  • Establish a supportive and effective working relationship with each Individual and their personal support network
  • Facilitate the person-centered planning or personal planning process with Individuals upon request.
  • Complete documentation as required by BHSSA policy
  • Consider and promote the opportunity for growth and independence of Individuals in the development of Individual Person-centered plans.
  • Promote the development of natural community supports
  • Act as a role model, displaying professional attitudes and behavior.
  • Promote the Individual’s health and well-being, i.e. exercise, healthy meal choices, self-management skills (i.e. time management, stress management, work related social skills)
  • Facilitate the service planning process for Individuals, ensuring updates, assessment information and other relevant information is collected and distributed prior to the service planning date.
  • Review the Individual Profile Registry on a weekly basis for potential clients.
  • Meet with Program Directors and Executive Director to determine potential clients to pursue for services or to review any applications or referrals for services.
  • Complete Expressions of Interest (EOI) for identified potential clients.
  • Facilitate process for potential new clients including: Meet and Greet meetings, Intake meetings, information sharing, documentation gathering etc.
  • Provide resource information, referrals etc. for clients seeking services that may not fit admission criteria or seeking information on services.
  • Refer clients seeking ABII, CAPCC or BCAL services to the appropriate Service Coordinator.
  • Ensure all completed documentation, forms and information etc. is forwarded to the Coordinator of the program the new client is assigned to

b. Documentation

  • Ensure complete and confidential records are kept as per policy requirements.
  • Ensure all documentation is completed in a timely manner.
  • Collect and distribute all information to all relevant parties at least one week prior to the service planning conference.
  • Complete all required documents for new clients.
  • Complete and required documentation for discharge of clients.

c. Teamwork

  • Work effectively with Individuals, team members and others in a positive and professional manner.
  • Engage the Behavior Specialist and/or Program Coordinator as needed in the Admissions process.
  • Relay important concerns to supervisor in a timely manner.

d. Liaison

  • Establish and maintain effective and professional relationships with the Individual’s personal supports, including the individual’s guardian, family members, friends and natural community connections.
  • Establish and maintain effective, open and professional working relationships with other Blue Heron employees working in support of the Individual.
  • Participate in the exchange of relevant information with the purpose of establishing more effective utilization and coordination of services.

e. Other

  • Any other duties required from time to time as identified by the Program Director.
  • Submit suggestions and ideas that may improve the quality of life of the Individual or strengthen team operation to the Team Leader.
  1. Qualifications

a. Education

  • Degree or 2-year Diploma in Disability Services, Social Work or Human Services

b. Experience

  • Previous experience in facilitation
  • Knowledge of alternative methods of communication such as sign language would be an asset

c. Skills/Abilities

  • Excellent verbal and written communication
  • Superior interpersonal and leadership skills
  • Superior computer skills

Pay: $22.96-$34.30 per hour
