Description DUTIES: 1. Working directly with the individual, guardian, family, and County Board in attending ISP meetings. Being knowledgeable of the services RVI provides. 2. Gathers data for outcome tracking system 3. Gathering information from the Department manager and DSP to take to the ISP. 4. Provide communication (both verbal and written) between employees, site management, RVI staff, guardians, other providers and County Boards. 5. Attend individuals annual ISP meeting to represent RVI INC. and services we can provide. 6. Review and verify the ISP for services, coding, outcomes, verbiage, and PAWS for RVI as the provider. 7. Working directly with team to create and write person centered skill developments and outcomes for RVI. 8. Relay meeting information and approved ISP to DSPs, team leads, and department directors. 9. Set up of individual's documentation in BID and paper. Offer documentation training when needed. 10. Completes all required paperwork, efficiently, and accurately. 11. Participate in professional development including training and seminars. 12. Collecting data of individual's goals monthly, quarterly and annually, to continue to work on person centered planning and personal development. 13. Assisting in collecting data for OTS and entering information into the system. 14. Identifies and relays concerns about individuals we serve to Department Managers and Operations Director. 15. Provides instruction and/or consultation in a variety of areas: home, work, community and transportation 16. Provides guidance to individuals and families as necessary. 17. Participates in the development and expansion of community resources to address individual needs. 18. Responds to requests for information or referrals. 19. Implements and maintains a variety of documentation for service reimbursement, reports, records, correspondence, and other communication. To maintain RVI's compliance with DODD, CMS, OOD, and any other compliance needs. 20. Implements skill development and outcome training objectives for individuals in a variety of settings. 21. Enter new and renewed ISP information into Software Systems as needed. 22. Provide staff training on ISP's and individual specific training to RVI staff, to maintain compliance standards. 23. Be the contact person for Documentation Systems to assist staff. 24. Reviews incident and injury reports, assisting staff with writing reports following RVI reporting procedure. 25. Report safety violations or concerns to Safety Committee using the appropriate form. 26. Practice "Universal Precautions" (i.e. wear latex or similar gloves) according to required training procedures. 27. Performs other tasks as deemed necessary by Operations Director 28 Work directly with co-workers to cover daily and/or weekly requirements as needed. 29. Assisting and contributing in Compliance reviews 30. Participates in required committee meetings and training opportunities. 31. Provides information and performs internal compliance reviews. 32. Incident reporting/MUI reporter 33. Embody Trauma Responsive Care practices Requirements SKILLS AND QUALIFICATIONS: 1. High school diploma or equivalent. 2. Habilitation programming for individuals with IDD. 3. Ability to complete mandatory IDD certification classes. 4. Completion of mandatory Blood Bourne Pathogens, CPR, and First Aid training. 5. Deal with problems involving several variables in familiar context. 6. Maintain accurate records. 7. Willingness to work with and cooperate with coworkers. 8. Ability to perform personal service duties, including but not limited to, feeding and bath rooming. 9. Valid drivers license. 10. Excellent written and verbal communication skills. 11. Excellent organizational skills and proficiency in professional office procedures. 12. Prefer 2 years experience in IDD field, equivalent education, or combination of both.
