About the role
Services Relationship Coordinator – Primary Care works within the Commissioned Services Team and collaborates closely with CHN-funded health service providers in this key contracts, evaluation and relationship management role. This position also works closely with other key internal and external stakeholders to facilitate improved health outcomes across the ACT region in the primary care program area.
About the team
Based on the needs of the ACT community, Capital Health Network (CHN) commissions innovative and evidence-based services for people at-risk of poor health outcomes. Reporting to the Service Relationship Manager - Primary Care, this position will work within the Commissioning Team that is responsible for the commissioning of both primary care and community services. The position collaborates closely with CHN-funded health service providers across priority areas, including mental health, aged care, alcohol and other drugs, and people at-risk of poor health outcomes.

You’ll work closely with stakeholders within CHN and externally to contribute to program co-design activities and to develop and manage health services contracts that meet the requirements of our funding agreements, such as the Department of Health, Disability and Ageing and the ACT Health and Community Services Directorate. This will support service delivery and attainment of health outcomes across the ACT region that meet the needs of populations identified in CHN’s health needs assessments. Your strong relationship management skills, attention to detail and contract management capabilities will enable you to support the delivery of primary health programs to make a real difference in the community.
As part of working at an innovative organisation, you will also contribute to process improvement and may have an opportunity to work on broader CHN projects and initiatives, including contributing to improved collaborative commissioning outcomes over time that support the efficiency, cost-effectiveness and value of CHN-funded health programs.

This position is based in our newly renovated and modern office, located in the heart of the Belconnen town centre. We support flexibility and a hybrid work model at CHN and are happy to work with the successful candidate to ensure balance between work and life.

Position Salary: $49.73 per hour (approximately $98,582 per annum, pro rata to full-time equivalent)
Employment type: Full time (38 hours per week), fixed term until 30 June 2028
Working Days/hours: Full time standards, negotiable hours
