Description
Responsibilities
Provide client-centred settlement information and referral services including assessment, information and orientation services for immigrants and refugees at other community partners’ locations
Use initial assessment of newcomers to develop personalized settlement plans
Provide case management and supportive follow-up services
Facilitate appropriate internal and external referrals and access to services in the community
Develop and maintain positive working relationships with host agencies
Build partnerships with other service providers to connect clients with appropriate resources
Organize, plan, facilitate group sessions as required by contract, and are in line with client needs
Develop and implement community outreach plans to promote CCS programs and services
Participate in regular program, team and regional meetings
Collaborate with manager to develop and implement initiatives and/or focus activities to address newcomer needs. (e.g. financial literacy, youth, women)
Maintain and update all client information in customized database
Prepare and submit statistical and progress reports to supervisor as required
