QUALIFICATIONS:

  • BSW or MSW degree; with 5 years related experience, two of which must be working with street involved youth
  • Registration in good standing with the College of Social Workers and Social Service Workers
  • Strong commitment to integrated model of practice (client centred, AOP/ABR, trauma informed, harm reduction, Housing First for Youth, strengths/resiliency model based).
  • Demonstrated expertise in case management and effective crisis intervention with youth who are experiencing complex challenges related to homelessness such as equity and access needs, harm reduction strategies, mental health support, in addition to their basic needs.

Responsibilities
  • Ensuring ongoing case management engagement for youth assigned to Case Manager and their Case Management Team
  • Meeting with youth to collaboratively assess their needs, discuss plan options, and develop a case plan that meets youth’s needs and desires, reflects youth’s strengths, and enhances resiliency and positive youth development.
  • Assessing risks; develop and support safety plans with youth (eg. relational, mental health, substance use concerns); coordinate proactive risk mitigation strategies.
  • Overseeing collaborative implementation of Case Plan support steps with Case Management Team members and other agency staff; monitoring, evaluating and mentoring interventions as needed to actively help support youths’ progress.
  • Ensuring case management support includes appropriate resources, referrals, and facilitated access to internal and external service providers to meet both case management and clinical needs of youth (eg. related to physical/ mental health/ substance use/ cognitive needs)
  • Conducting regular case plan reviews, updating documentation of progress, next steps, and goal revision, by directly reviewing with youth, consulting with designated Consistent Workers and others in circle of care; chairing Case Management Team review meetings.
  • Coordinating case conferences and consultations with internal and external client supports as needed
  • Knowledge of community resources for mental health and substance use
  • Knowledge of community resources for housing, supportive housing and shelter
  • Ability to work closely with teammates and participate effectively as part of the larger program and organization
  • Ensuring all case documentation/client files, agency program documentation, and data collection are completed and maintained according to agency and professional standards
  • Contributing towards an evidence based, Quality Improvement model of practice.
  • Actively participate in the self-development of clinical case management skills
  • Participate in program and interdepartmental meetings, and supervision as required.
