We are immediately seeking a PART TIME or Per Diem Speech Language Pathologist SLP Includes a $5,000 Signing Bonus and would love to work with you in our Home Health department! Territory: North and East (King County) Primary Responsibilities: Meets the resident's needs and provides quality care by assessing and interpreting evaluations and determine treatment plans in consultation with physician. Implements treatment towards goals indicated in treatment plan. Initiates and completes discharge planning by consulting with physicians, nurses, social workers, and other health care professionals. Assures continuation of therapeutic plan following discharge by designing resident specific maintenances programs and/or referring for outpatient or home health services. Works as an active member of the therapy team to provide collaborative treatment services. Organizes and prioritizes daily treatment schedule. Electronically maintains thorough and accurate documentation of therapy services and patient progress. Prints and files all paperwork in a timely manner. Completes therapy minutes in MDS. Completes weekly PPS assessment forms. Secures and maintains equipment and supplies. Qualifications Graduate from an accredited program for Speech-Language Pathology Home Health experience preferred Current license as a Speech-Language Pathologist in the state of WA Must be able to communicate in English, both verbally and in writing Familiarity with clinic and patient related software Proficient with MS Office Suite, particularly Word and Outlook
Responsibilities
The Speech Language Pathologist will assess patient needs, develop treatment plans, and implement therapy to help patients regain independence. They will also collaborate with healthcare professionals and maintain accurate documentation of therapy services.
