Job Details Job Location: OMAHA, NEBRASKA - OMAHA, NE Education Level: High School Salary Range: Undisclosed Travel Percentage: None Description Salary $45,000 - $50,000 a year This position works closely with individuals and families throughout Nebraska, coordinating supports and services for existing and new placements. This position will work collaboratively with the Shared Living coordinator to provide services in accordance with the Agencys mission and vision. CASE MANAGEMENT SERVICES: Oversee the person-centered program planning through the supervision, development, and management of client. Complete necessary documentation in a accurate and on time. Assist clients in the identification of strengths, needs and goals. Train staff (possibly in interaction strategies), as requested, in methods and strategies, and monitors for implementation and effectiveness. Ensure that all regulations are adhered to. Performs med administration, as assigned. SUPERVISION OF HOME: Participate in the development of a new home for assigned clients. Provide training, oversight, and support to the assigned contracted home providers who are housing clients. Ensure adherence to regulations and the contract by having professional and transparent conversations with home provider. Review and process monthly documentation in Electronic Medical Record System. Visit the home minimally one time per month or more often if needed. Responsibilities: Under the guidance of the Shared Living Coordinator, this position will have responsibility for the development and supervision of individual home share placements. Coordinate services including conducting home visits, coordinating treatment with Service Coordinator and other care providers, attending Service Planning meetings and documentation of all Case Management activities. Maintain program compliance with all applicable state funding agencies, certification regulations and requirements and quality service standards Coordinate services that promote and facilitate a person-centered approach based on best clinical practice Provide training, supervision and evaluation of providers as assigned. In collaboration with the Shared Living assist in the referral, intake and placement process Maintain compliance with all applicable state and federal funding agency certification regulations and requirements, as well as quality service standards. Perform other job-related responsibilities as assigned. Qualifications High School diploma or GED required; Minimum of one year related work experience; supervisory experience preferred; Must be 18 years of age Valid drivers license in good standing; car registration and auto insurance if driving on the behalf of the Company or providing transportation to individuals receiving services. Successful clearance of local and/or state background checks A strong work ethic with a commitment to serving others Excellent communication skills with an ability to establish rapport with team members and those we serve Strong organizational abilities to ensure staffing and schedules are maintained
