Responsibilities: * Conduct comprehensive assessments of members’ mental health status, social needs, and functional abilities to inform individualized care planning. * Develop, implement, and regularly update personalized care plans in collaboration with members, families, and healthcare professionals. * Coordinate access to medical, psychiatric, and social services, ensuring timely referrals and follow-up. * Monitor member progress through regular contact, adjusting care plans as needed to address changing needs or challenges. * Advocate for members’ rights and needs within healthcare systems and community resources to promote equitable access to services.
