Job Details Job Location: Westerfield Building PC 0AV58 - Hopkinsville, KY 42240 Position Type: Full Time Education Level: 4 Year Degree Salary Range: $15.87 - $15.87 Hourly Travel Percentage: Up to 50% Job Shift: 1st shift Job Category: Case Manager PRIMARY RESPONSIBILITIES: This employee will provide case management services to adults with a diagnosis of a severe mental illness. Duties include: (1) Provide intensive case management to identified clients. (2) Have ongoing contact with community agencies necessary to develop additional resources and coordinate services. (3) Assist in developing and accessing housing for clients. (4) Must be able to establish rapport with clients and families. (5) Must have transportation and be willing to assist clients by providing outings and keeping scheduled appointments with various community advocacy agencies. (6) Employee is also responsible for timely record keeping associated with the services provided. (7) May fill in from time to time during the absence of other staff, as assigned by supervisor. (8) Attend one (1) individual and one (1) group supervision monthly as scheduled by supervisor. (9) Complete and maintain documentation of a minimum of six (6) hours of training or continuing education each year. (10) Other duties as assigned/required. Qualifications Employee is required to have at least a bachelor's degree in psychology, social work, or related field with at least 1 year of experience in working in a human services setting. Must have a valid driver's license, reliable transportation, and be willing to transport clients on a daily basis. Must present proof of automobile insurance and pass a Criminal Record Check. Within 6 months of employment, this employee must attend the Core and SMI case management trainings.
Responsibilities
The SMI Case Manager will provide intensive case management services to adults with severe mental illness. Responsibilities include coordinating services, developing housing options, and maintaining client records.
