Description
The SNF LVN is responsible for the assistance in operations of the Quality Assurance department. Responsible for obtaining data/metrics to facilitate our financial analysis of hiring for expansion and growth. Essential Job Functions · Manage and maintain a spreadsheet of provider/patient issues, including all emails and phone calls necessary for resolution. · Audit of all D/C charts before patient is discharged. · Assist with provider issues that come in from all departments. · Scheduling of all labs/ultrasounds/x-rays. · Clinical audits on active patients · Monitor wound cultures and antibiotic prescriptions. · Provide results of biopsy, skin scrapes and other lab results to the provider and uploading of all results to patient charts. · Handle all referrals – Home health, hospice, dermatology, and PCP. Qualifications · At least 1-2 years of SNF experience preferred · Knowledge of safety requirement and legal standards · Strong leadership skills · Strong written and verbal communication skills · Excellent collaboration and product management skills and experience · Current California LVN Job Type: Full-time Benefits: 401(k) 401(k) matching Dental insurance Health insurance Life insurance Paid time off Vision insurance Work Location: In person
