NP GERIATRIC - JOB DESCRIPTION

Summary: We are seeking a dedicated and skilled Nurse Practitioner to join our healthcare team. The ideal candidate will possess a strong clinical background and demonstrate a commitment to providing high-quality patient care across various settings. This role requires proficiency in both acute and chronic care management, with an emphasis on patient assessment, treatment planning, and collaboration with multidisciplinary teams.
The NP provides medical management of the residents in coordination with attending physicians, nursing and rehabilitation staff. The NP provides clinical support and communication to residents, families, the interdisciplinary team and nursing center’s administration.

PROFESSIONAL EXPERIENCE/EDUCATIONAL REQUIREMENTS

  • Registered nurse who is mastered prepared as an advanced practitioner
  • A minimum 2 years clinical experience as a registered nurse and 2 years as an APRN is preferred.
    Geriatric nursing experience is a plus

Responsibilities
  • Makes visits to residents in skilled nursing facilities and/or assisted living facilities to provide medical care and treatment to their residents;
  • Performs physical examinations and preventive health measures within prescribed guidelines and instructions of attending physicians utilizing approved protocols;
  • Orders, interprets and evaluates diagnostic tests to identify and assess residents’ clinical problems and health care needs;
  • Is responsible to ensure documentation is complete and timely for resident management/continuity of care and billing purposes. Documentation of medical necessity for purposes of billing visits;
  • Is responsible to ensure communication of billing information to the billing service;
  • Ensures that prescriptive authority and order writing are performed in accordance with applicable laws and standards;
  • Develops understanding of facility programs and policies/procedures/guidelines;
  • Communicates with interdisciplinary team and Director of Nursing as well as medical staff to ensure continuity of care delivery;
  • Monitors quality indicators, identifies areas of opportunity and seeks solutions to enhance care and outcomes; and
  • Is a resident advocate and communicates with families when appropriate.

The Nurse Practitioner:

  • Is a resident advocate;
  • Performs annual and routine exams as well as episodic and acute care visits when needed;
  • May visit a resident during the admission process, when there is a medical condition which requires immediate assessment;
  • Performs exams as relates to change in condition;
  • Performs exams as relates to medication review and changes;
  • Performs exams pursuant to collaboration with resident’s physician to manage resident’s medical care;
  • Provides clinical monitoring during course of treatment;
  • Completes review of physician orders for medical necessity, supporting diagnosis, pharmacological contraindications, generic vs. trade or OTC;
  • Participates in psychotropic medical management and behavior management process;
  • Maintains prescriptive authority;
  • Provides clinical monitoring;
  • Provides ongoing clinical management;
  • Ensures documentation is complete, and meets medical necessity and billing guidelines;
  • Participates in the survey process;
  • Understands facility clinical systems and processes;
  • Completes Medicare certifications as necessary;
  • Must maintain active licensure status and current continuing education credits as required by law;
  • Must have valid DEA License with all schedule prescription authority under guideline of supervising physician.
  • Performs other duties as assigned including education and quality management support; and
  • May provide services such as employee physicals and in-services pursuant to facility agreement.

Compliance Responsibilities The Nurse Practitioner:

  • Complies with applicable legal requirements, standards, policies and procedures including but not limited those within the Corporate Compliance Program, Corporate Code of Conduct, HIPAA, and Federal False Claims Act.
  • Attends the center’s orientation and training programs as required;
  • Reports concerns and suspected incidences of non-compliance immediately to Medical Director or Chief Compliance Officer.
  • Cooperates with monitoring and audit functions and investigations; and
  • Participates, as requested, in process improvement responsibilities.
