THE OPPORTUNITY

Melton City Council is seeking a Social and Client Connections Worker to support older people and individuals with disabilities in maintaining their independence, wellbeing, and social connections. You will deliver high-quality community care and social support under the HACC-PYP and CHSP programs, working both individually and as part of a team to create a welcoming and engaging environment for clients.

Responsibilities

KEY RESPONSIBILITIES

  • Provide community care support including domestic assistance, personal care, flexible respite, social support individual, centre and community-based support, and other support services, inclusive of mobility assistance to eligible HACC PYP and CHSP clients using a person-centred approach as outlined in their care plans.
  • Support clients to maintain and develop independent living skills and routines.
  • Foster positive and respectful relationships with clients, respecting cultural values, beliefs and individual choices.
  • Monitor the physical, social, and emotional wellbeing of clients and report any concerns to the respective Supervisor.

TO BE SUCCESSFUL IN THIS ROLE YOU WILL HAVE THE FOLLOWING:

  • Certificate III in Individual Support (Ageing, Home and Community, Social Support Individual) or equivalent and/or experience in a relevant field that demonstrates capability to undertake theposition.
  • Experience, knowledge and skills relevant to the various community care support services provided to eligible clients under the HACC-PYP & CHSP.
  • Experience, knowledge and understanding of the needs of older people, people with disabilities and their carers.
  • Current valid Victorian Driver’s Licence and access to a safe and appropriately registered, comprehensively insured vehicle suitable for the purpose of transporting clients.
  • Must hold or be able to obtain an Employee Working with Children Check.
  • Current Level II First Aid Certificate, including CPR module.
  • Available to work between Monday and Friday 7am and 7pm, weekends, public holidays and irregular hours as service requires.
