As a Social Care Practitioner, you will be at the heart of improving lives and wellbeing across Wiltshire. Working collaboratively with Occupational Therapists, Social Workers, and multi-professional teams, you will promote independence, choice, and wellbeing using universal services and community resources. As this is a fast-paced team, you will lean on your organisational ability to manage caseloads effectively and have excellent communication skills to be able to work collaboratively. In addition to a sound understanding of the Care Act 2014, you will hold an NVQ Level 4 in Health and Social Care or equivalent or the ability to work towards this qualification and have experience of working within a Health or Social Care setting. You will be providing outcome-focused assessments to identify the right support for positive life changes and creating and reviewing support plans that make a real difference.
