WHAT WE DO:
Daffodil Care Services has developed a nationwide network of aftercare services consisting of both residential and comprehensive outreach services. Our residential aftercare services are fully staffed at all times providing timely support to vulnerable young adults as they progress to independent living. Our aftercare teams are dedicated to ensuring young adults get both the support and experience required in preparation for independent living.
THE ROLE:
This is a dynamic service, providing individualised interventions to young adults aged 18 – 23 year olds, to enable them to live in safety and with stability and support to begin their journey to independence. Working as a Social Care Worker you will support guide and lead by example the vulnerable young people in our care, in conjunction with the wider multidisciplinary team involved in their care.
You will ensure the welfare of those under your care and will act as an advocate as appropriate.
The Social Care Worker will contribute to the planning and evaluation of individualised placement plans, which are based on strengths and needs and delivered through day-to-day shared life experiences within the residential aftercare setting. The service will provide support to ensure their safety, promote their stability, and build resilience to enable them to develop their independent living skills. You will work as a lone worker but part of a team to provide a safe, caring and trauma informed environment for the residents with the primary aim of providing the interventions necessary to aid them moving into full independent living.
You will support and empower vulnerable young adults to identify goals and person-centered targets while working through the process of gaining independence.
