Responsibilities

JOB PURPOSE:

To coordinate delivery of the social prescribing service

PURPOSE OF THE ROLE:

The successful candidate will be partly responsible for the operational delivery of the community based Social Prescribing service, receiving referrals from a range of referrers including GPs and other Healthcare professionals in the East Kent Area. The role supports individuals to access a range of health, social care, welfare advice and other support services with the aim of improving their physical & mental health and overall wellbeing.
The role will require proactive engagement with other professionals and patients to stimulate referrals into the programme and with community-based providers of services and activities. Individuals will be supported on a person centred basis to identify activities that will improve their health and wellbeing. Building great relationships and enhancing our existing connections with other organisations is key to connecting people to the social activities they want and need.

ADDITIONAL RESPONSIBILITIES

  • Adhere to Red Zebra’s policies and procedures at all times including equality and diversity, health and safety, confidentiality, lone working and safeguarding
  • Ensure that the services provided reflects the vision, mission and values of Red Zebra
  • Assume responsibility for producing own correspondence, performing and maintaining administrative tasks
  • Undertake relevant internal and external training
  • Maintain professional knowledge by keeping up to date with current legislation etc
  • Undertake any other additional tasks as reasonably deemed appropriate by the social prescribing service
  • Be an advocate for Red Zebra as well as the people we work with
