SOCIAL SERVICE DIRECTOR

Wellington Parc Nursing and Rehabilitation is seeking a dynamic Social Service Director.

REQUIREMENTS

  • Bachelor Degree in Social Work or related field.
  • Certified as required by the state of employment.
  • One year of social work experience in a long-term care environment.
  • One year supervisory experience in a setting service setting

Responsibilities
  • Supervise the social services department in the day-to-day operations ensuring to set job assignments and set priorities.
  • Develop goals and objectives for the department; motivate and manage care team members to accomplish the same.
  • Ensure new care team members are effectively oriented; ensure an ongoing educational program is in place.
  • Develop positive relationships on behalf of the community with regulatory agencies, residents, and families including participating during family orientation and family council.
  • Assist in preparing budget for the department; administer approved budget.
  • Implement policies/procedures with follow-up and supervision of care team members to ensure compliance.
  • Provide direct psychosocial intervention.
  • Perform resident assessments at admission, upon condition change, and/or annually.
  • Create, review, and update care plan and progress notes.
  • Provide direct psychosocial intervention.
  • Coordinate resident visits with outside services, dental, optical, etc.
  • Attend and document resident council meetings.
  • Assist resident families in coping with skilled nursing placement, physical illness, and disabilities of the resident as well as the grieving process.
  • Work with the resident, family, and other care team members to plan discharge.
  • Conduct in-service programs to educate staff regarding psychosocial issues and patient rights.
  • Perform other tasks as assigned.
