We’re looking for a skilled and compassionate Social Service Workers to join our Integrated Care Solutions team. Part-Time position available: Part-time requires 2-3 days of daytime/weekday. Areas of service consist of Owen Sound and surrounding areas. Per visit rate and mileage as this is a home care position. Personalized Care: Develop and implement care plans tailored to meet the unique needs and preferences of each client. Collaborative Approach: Work closely with clients, their families, caregivers, service providers, and other professionals to deliver effective and respectful care. In this role, reporting to the Clinical Manager, you’ll be a key part of a care strategy that combines expertise with empathy. Your contributions will ensure that our clients receive the highest quality care and support, whether they are young or elderly, and regardless of their socio-economic situation. Duties and Responsibilities Implement Care Plans: Execute social and wellness care plans under the supervision of a Licensed Social Worker. Provide in-person and virtual support for wellness checks, food and housing security, and social connections. Community and Resource Management: Follow up on community service referrals, manage patient transportation, and advise clients on available resources. Facilitate referrals and assist in discharge planning when social work intervention is no longer needed. Caseload and Record Management: Efficiently manage caseloads and maintain treatment records in compliance with the Ontario College of Certified Social Workers and Social Service Workers standards. Client Support and Education: Use a client-centered approach to address psychosocial issues, provide relevant educational resources, and participate in quality improvement initiatives. Professional Development and Collaboration: Engage in ongoing professional development, maintain confidentiality, and build positive relationships within the interdisciplinary healthcare team. Adapt to a dynamic environment and perform additional tasks as assigned. Education and Certifications Required: Social Service Work diploma Current Member (in good standing) of the Ontario College of Social Workers and Social Service Workers. Current CPR certificate (Basic Rescuer) Clear Criminal background (Police) clearance with Vulnerable sector screening Valid driver's license, auto insurance and reliable vehicle & must carry two million dollars in automobile insurance coverage if utilizing a personal vehicle for appointments. Experience: Possess general computer skills with access to a computer with appropriate password protection and internet capabilities. Minimum of two years of community-based therapy experience (preferred). Other Skills and Abilities: Exceptional interpersonal skills; Ability to work independently and lead a team; Excellent report writing and communication skills; Able to remain calm under pressure and role model composure Solid knowledge of the principles, practices, and methods of community care and service delivery.
