Partners Community Health’s Integrated Seniors Hub is dedicated to providing comprehensive and compassionate care for seniors, ensuring they live their lives with dignity, independence, and purpose. Our seniors’ hub offers a holistic approach to senior care, integrating a variety of community-based services and programs designed to address the physical, emotional, social, and nutritional needs of our clients.

POSITION OVERVIEW:

We are seeking a compassionate, organized, and experienced Social Service Worker (SSW) to join our multidisciplinary team. The successful candidate will play a key role in supporting seniors and their families through care coordination, advocacy, and conducting RAI CHA (Resident Assessment Instrument – Community Health Assessment) evaluations.

QUALIFICATIONS:

Education

  • Diploma or degree in Social Service Work, Social Work, or a related human services field.
  • Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or relevant provincial regulatory body.
  • RAI CHA Certification (an asset) and demonstrated experience administering the assessment.

Qualifications & Experience

  • Minimum 2 years of experience working with older adults in community, hospital, or long-term care settings.
  • Strong understanding of the social determinants of health and community resources for seniors.
  • Exceptional communication, organizational, and interpersonal skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Proficiency in electronic documentation systems and Microsoft Office.
Responsibilities
  • Conduct RAI CHA assessments to support care planning, eligibility for services, and ensure best practices in client-centred care.
  • Collaborate with nurses, physicians, PSWs, and allied health professionals to develop individualized care plans.
  • Provide emotional support, crisis intervention, and resource navigation to seniors and their families.
  • Collaborate with Mississauga Health Navigators to assist in system navigation, including referrals to appropriate health, community, and social support services.
  • Participate in case conferences, inter-professional team meetings, and client reviews.
  • Maintain accurate, timely documentation in accordance with organizational policies and professional standards.
  • Plan and support health promotion initiatives and wellness programming within the hub.
  • Advocate for the rights and needs of seniors, including culturally sensitive and trauma-informed care.
