City Creek Post Acute is seeking a social services assistant to join our amazing team! We are a 75 bed facility located here in the heart of Downtown Salt Lake City.
Hours: Full-Time
Job Responsibilities:

The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department in accordance with current federal, state and local standards, guidelines, and regulations, our established policies and procedures, or as may be directed by the Director of Social Services and/or Administrator.

  • Coordinates and implements the function of discharge planning for patients with in the facility
  • Provide ongoing support and expertise through comprehensive planning, implementation and overall evaluation of individual patient needs
  • Enhance the quality or patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and management and discharge planning
  • Coordinates the integration of the social services function into patient care
  • Adhere to departmental goals, objectives, standards of performance and policies and procedures
