Description Social Services Assistant Full Time. Vineyard Hills Health Center, a skilled nursing facility in Templeton, CA , is looking for that special person who has a strong desire to advocate for our residents. We are hiring for an Assistant looking to grow and be part of a our team. Our Social Service Assistant, help residents with their basic needs of coordinate and act as representatives for seniors who are unable to advocate for themselves. This is a full-time position. If you have a passion to care for seniors, this is the right opportunity for you! The Social Service Assistant provides administrative and client support to social workers and case managers. This role assists individual and families in accessing social services and resources maintaining accurate records. Ensuring timely communication, and follow up. The ideal candidate is compassionate, organized, and committed to helping vulnerable populations navigate support systems. Conducting admission interviews, inform residents and their families of their rights, advocate for residents with social services organizations, and help residents to adjust to living inside and outside the facility and coordinate care services. Requirements What You'll Bring: Compassion for the elderly and commitment to hospitality and service. Strong communication and interpersonal skills Empathy and patience Organizational and recordkeeping ability Basic understanding of social service systems and confidentiality laws. High School Diploma. Positive and nurturing personality, a true want to help and care for those in need. Experience in long-term, skilled nursing care a plus, but not required. Open and flexible schedule, availability to work in the evenings or during weekends if needed. Proficient with Microsoft Office. Requirements Standing, lifting, twisting, bending, walking for long periods of time. Must be able to lift 50 pounds. Applicant must hold a valid drivers license.
