Requirements MUST HAVE SNF EXPERIENCE AND DISCHARGE/SOCIAL SERVICES EXPERIENCE IN ORDER TO APPLY. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
