Why does the job exist?
This position is an internal coordinator who works with the external case manager and all members of the Interdisciplinary Team to assure the presence and availability of services and provides support for New Mexican’s with developmental disabilities placed into Community Living Services with the Los Lunas Community Program.
How does it get done?
The key responsibility of this position is to work with external case managers and all members of the Interdisciplinary Team to assure all services and support are implemented for each individual served
Who are the customers?
This position provides customer service to all internal and external customers who support the Individuals Served at LLCP, this includes but is not limited to the following: all staff members, individuals served, team members.
Ideal Candidate
This position provides customer service to all internal and external customers who support the Individuals Served at LLCP, this includes but is not limited to the following: all staff members, individuals served, team members.
Minimum Qualification
High School diploma or equivalent and two (2) years of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling two (2) years may substitute for the required experience.
Employment Requirements
Must possess and maintain a valid New Mexico Driver’s License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive a Defensive Driving Course Certificate as a condition of continued employment within 45 days of employment. Successful completion of the Health Insurance Portability and Accountability Act (HIPAA)
Working Conditions
Office setting with extensive use of personal computer, telephone, fax, copier and other related office equipment that involves sitting, standing, bending, reaching, etc. to include lifting up to 30 pounds.
