Responsibilities

THE SOCIAL SERVICES COORDINATOR IS RESPONSIBLE FOR:

Supporting the DDSDs provider network which includes over 12,000 active users;
These users consist of internal DDSD staff, consulting clinicians, administrators from providers, case management agencies, and personnel within the Health Care Authority

THESE KEY RESPONSIBILITIES:

User Account Management, creating and maintaining user accounts, granting appropriate access, and ensuring secure entry into the Therap system;
Technical Support and Troubleshooting, assisting users with system navigation, resolving access issues, and troubleshooting record visibility across the Therap network;
Training and Guidance, providing one-on-one training and technical assistance to both internal oversight users and external provider administrators to ensure effective use of the system;
Data Coordination, managing user maintenance data and ensuring accurate documentation across systems;
Interdepartmental Liaison, collaborating with other units within DDSD to support system integration, data sharing, and operational consistency;
Ensuring that users across the DDSD network can reliably access and utilize the Therap electronic health record system to document and deliver high-quality services to individuals with developmental disabilities.

8,500 individuals with developmental disabilities in New Mexico enrolled in waiver programs;
A broad network of professionals and stakeholders including DDSD staff, other divisions within the Health Care Authority, provider and case management agencies, consultant clinicians, guardians, and external partners;
The infrastructure essential for delivering safe, person-centered, and coordinated care across the DDSD network.
Ideal Candidate
8,500 individuals with developmental disabilities in New Mexico enrolled in waiver programs;
A broad network of professionals and stakeholders including DDSD staff, other divisions within the Health Care Authority, provider and case management agencies, consultant clinicians, guardians, and external partners;
The infrastructure essential for delivering safe, person-centered, and coordinated care across the DDSD network.
Minimum Qualification
High School diploma or equivalent and two (2) years of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling two (2) years may substitute for the required experience.
Employment Requirements
Must possess and maintain a valid New Mexico Driver’s License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive a Defensive Driving Course Certificate as a condition of continued employment within 45 days of employment. Travel will be required with overnight stays. Successful completion of the Health Insurance Portability and Accountability Act (HIPAA) and other trainings as required.
Working Conditions
Office setting with extensive use of personal computer, telephone, fax, copier and other related office equipment that involves sitting, standing, bending, reaching, etc. to include lifting up to 30 pounds.
