Description SOCIAL SERVICES COORDINATOR JOB DESCRIPTION A. CLINICAL/ADMINISTRATIVE: Ensures the residents' psychosocial and concrete needs are identified and met in accordance with Federal, State, and Company requirements. Orients residents to the center. Discharge planning. Implements and updates Resident Care Plan and Social History. Maintains records of outside referral. Assists in coordinating residents' financial affairs. Communicates needs and Care Plans to resident, family, responsible party, and appropriate staff. Participates in meetings, in-services, and training as required. Meets with consultants as required. Organizes and facilitates Family Group. Organizes and conducts New Admissions Group. B. CONSUMER SERVICE: Presents professional image to consumers through dress, behavior, and speech. Adheres to Company standards for resolving consumer concerns. Ensures that all consumer and patient rights are protected. Requirements C: QUALIFICATIONS: In Centers with 120 or more beds: - A Bachelors Degree in Social Work or a human serviced field; and- One year of supervised social work experience in a healthcare setting. In Centers with less than 120 beds: - High School Diploma or equivalent. In all Centers: - Neat in appearance;- Good written and verbal communication skills;- Good interpersonal skills;- Good administrative and organizational abilities;- Knowledge of Social Resources in the community;- Degree in Psycho/Social Health preferred.
Responsibilities
This role involves ensuring residents' psychosocial and concrete needs are met according to regulations, which includes orienting residents, managing discharge planning, and implementing/updating care plans and social histories. The coordinator also communicates care plans to residents, families, and staff, and facilitates necessary group meetings.
