POSITION SUMMARY:

Will collaborate with all staff for the care of behavioral health patients both inpatient and outpatient needs of the patient, the facility, and clinic.

POSITION REQUIREMENTS:

  • RN, LPN, or Bachelor’s degree preferred
  • Experience in social work, psychology/psychiatry, and/or nursing
  • Experience with behavioral health and/or case management preferred.
  • BLS within 6 months of hire

Responsibilities
  • Will celebrate with multidisciplinary team to help meet the needs of individual patient, especially when coordinating social disparity needs of the patient.
  • Provides psychosocial assessments for inpatient and outpatient referrals, i.e. assess patient’s social, emotional, environmental, financial and support needs.
  • Demonstrate some knowledge of medical terminology.
  • Implements interventions during crisis situations for the emergency department and critical care unit.
  • Collaborates with physicians, nurse practitioners and other providers regarding a patient’s care.
  • Provide patients with recommendations and provide referrals to other departments or community agencies as needed.
  • Facilitate the hospital’s bereavement, cancer and caregiver support groups as needed.
  • Complete assessments and documentation in a timely manner.
  • Assist with discharge planning as needed for outpatient referrals.
  • Advocate for patient’s human rights to be acknowledged in all decision-making.
  • Help patients/families adjust to hospital admission; possible role changes; exploring emotional/social responses to illness and treatment.
  • Effectively communicates with physicians, admitting office, medical records, staffing coordinator, and nursing staff to initiate patient care and enhance positive outcomes.
  • Assists with hospital and community education with clinical issues and participates in community events.
  • Works conjunction with a multidisciplinary team assess patient’s condition, formulates and implements individualized treatment plan and serves as a clinician and case manager.
  • Maintains knowledge of current empirically supported treatments and provide coverage for personal growth and psycho education groups when needed.
  • Handles incoming and outgoing communication with patients.
  • Responsible for keeping charting up-to-date.
