Description We are seeking a compassionate and dedicated Social Services Coordinator to join our team. Job Summary: To provide support and resources needed to enhance the well-being of the residents, while collaborating with their families and loved ones to improve resident quality of life. To collaborate with team members within and outside of Assisted Living to foster relationships, embrace multiple perspectives and improve outcomes. Education, Skills and Abilities: Education and experience equivalent to bachelor’s degree of social work. Minimum of 1-3 years of social work experience, preferably in a life plan community (CCRC). Proficiency in compassionately assisting older adults with the psychological challenges of aging Excellent communication skills with active listening Strong relational skills, able to build trust with team members, residents, families, visitors. Ability to work with confidential information and maintain its’ integrity. Proficiency in office equipment: computer, electronic medical record platforms such as PCC, other. Essential Job Duties: Coordinate admission, discharges and transfers by working with residents, POA’s, families, and team members within and outside of the department. Complete necessary paperwork and need for medical certificate, tb screen, chest x-ray or PPD for admissions. Collaborate with the Assisted Living Director, Nursing Coordinator, departmental team, and the Resident Care team to tour and support residents transitioning to Assisted Living. Provide information, education and support to residents, helping them adjust and embrace the day-to-day benefits Assisted Living brings to all dimensions of wellness. Problem solve with residents, team mates, and those involved in their care. Provide overall guidance for coordinating insurances and financial assistance. Assist residents with submission of insurance information, claims and documentation (written and phone calls) for long term care insurance. Connect residents with community agencies and resources as needed. Direct families to social security administration and provide information about representative payee. Work with team to ensure occupancy goals for Assisted Living are consistently met. Build an active futures list for internal residents and an external waitlist for those in the greater community. Regularly communicate with those individuals/POA’s. Coordinate with Assisted Living Director, Nursing Coordinator and departmental team to complete Individualized Service Plan. Visit residents on a regular basis in order to support the individual care plan. Document resident visits in the Electronic Medical Record. Schedule care plan meetings. Follow-up with families on who will be attending. Update all necessary reports, progress notes, and information for data entry and face sheets as needed. Update and maintain accurate Assisted Living census list and Assisted Living marketing report. Once per quarter, address advanced care planning, code status, Advanced Medical Directives and POA’s and ensure this has been documented in the EMR. Complete training and stay updated on the dynamics of cognitive impairment and dementia. Meet with new hires during their orientation to speak about the Social Services role in AL. Assist with investigation of complaints and stay abreast of Department of Social Services regulations. Helps ensure appropriate reporting to required regulatory agencies. Participate in education to staff on residents’ rights, abuse and neglect. – Provide training for new hire orientation and as needed. Facilitate shopping for residents by approving withdrawals from personal fund account. – Work with staff, residents, and families to see personal items and necessary clothing are provided. Contact families for permissions. Request items to be purchased and coordinate returns of items. Respond to inquiries for Assisted Living. Send out marketing and application materials to prospects timely. Maintain a prospect database; phone calls, tours, contact, etc. Keep a waiting list of received applications for Assisted Living and obtain approval from corporate. Oversee, maintain and complete Fellowship documentation monthly. Provide semi-annual education to all AL residents on Emergency Preparedness and have residents sign an acknowledgement. Update Assisted Living resident’s pictures annually for the EMR. Oversee and maintain elopement binders. Complete annual resident rights review with residents and have them sign acknowledgement. Complete annual sex offender screen information/education with residents and have them sign acknowledgement. Attend and provide info for quarterly resident council meetings and announce new resident move ins in the last quarter. Provide new admissions with a “Welcome Basket” to Assisted Living . Scan admission documents and drop into the residents EMR. Provide support to the Life Enrichment team, escort events when available, encourage and remind residents for activities. Cross train to provide Social Work and Admission support in HealthCare. Rotate weekend call when needed for admissions into Assisted Living/Healthcare. Collaborate with IL Clinic Director to establish relationships with IL residents who have an impending need to transition to Assisted Living. Assist residents and families with Mental Health resources with psychiatrist, counseling, etc. Ensure receptionist at both the Corson Lobby and the Highlands have the most current Assisted Living marketing information for distribution. Work with EVS and maintenance to see that vacant rooms are ready for showing and/or to move into. Place calls to family members and assist with the clearing of rooms.
