Responsibilities of the Social Services Director: Assist with planning, developing and directing our facility’s social service programs in accordance with current federal, state and local standards, guidelines and regulations Ensure that medical, emotional, and social needs of each resident is met/maintained Greet and orient new residents Provide information regarding community resources to residents' families Direct hospice and palliative care Coordinate discharge planning Facilitate regular care plan meetings with residents and their families Complete the MDS (Minimum Data Set) within the Assessment Reference date Address residents' grievances and assist nursing or other pertinent departments in resolving issues Attend/facilitate intervention, rehabilitation, marketing, performance improvement and Medicare meetings as appropriate IND1 Requirements of the Social Services Director: BA or Master’s Degree in Social Work (MSW) - or - BA in Psychology or Sociology A member of the National Association of Social Workers (NASW) and Academy of Certified Social Workers (ACSW) desirable Minimum two years' supervisory experience A minimum of three (3) years of experience in field of social services in a hospital, nursing or long-term care facility Demonstrated knowledge of the social dynamic of the geriatric population Knowledge of community resources Initiative to seek new methods and services for residents Strong leadership and relationship building skills High level of organizational professionalism Working knowledge of government regulations as relevant to social services “Generations - Our Family Caring for Your Family for Generations to Come” Equal Opportunity Employer
