THE OPPORTUNITY:

We are growing and expanding our Social Support Groups and are looking for Social Support Workers to join us. This is an exciting opportunity to join a team of passionate professionals who provide participant led group activities, outings and events while caring for the individual’s goals. You will play an important role in facilitating social support groups, offering one-on-one assistance and helping create a safe, welcoming and supportive atmosphere for all participants. Responsibilities include but are not limited to;

• Planning with participants/team and implementing a range of quality activities.
• Providing transport using work cars and bus (12 seater).
• Providing personal care support.
• Assistance with meal service as required.
• To be involved in review of care plans, progress notes, discussions with clients and family and update progress notes at the end of the day.
• Make contributions to the team.
