Serves as a resource within the social work department and the hospital. What You Will Do Provides psychosocial assessment / intervention to assist patients/families (including clinically complex). • Provides age specific care to patients/families in all age groups. Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs. • Develops plans with attention to individual patient and family goals • Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies. Communicates/collaborates with the work team using a multidisciplinary approach. Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers. Assists with recruitment, and orientation/mentoring/education of new staff and or social work interns. Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Education Master's Degree from an accredited school of Social Work (Required) Work Experience Hospital/Health Agency experience. (Preferred) Knowledge, Skills, & Abilities Computer skills (Required proficiency) Theoretical and clinical knowledge regarding health care delivery (Required proficiency) Multi-tasks and prioritizes work (Required proficiency) Works autonomously (Required proficiency) Communicates effectively with persons of various backgrounds (Required proficiency) Licenses and Certifications Licensed Social Worker (LSW) (Required) Licensed Independent Social Worker (LISW) (Preferred within one year of eligibility) Position Details/Preferred Qualifications At least 2 years of post-Master’s experience, preferably in pediatrics and with chronic disease processes, as well as have hospital experience. This Social Worker will care for pediatrics in outpatient Cardiology but would cross cover for the inpatient side. They will meet with the patients and their families, perform assessments, set up and facilitate care conferences, connect families with community resources, perform discharge planning, etc. Position is for day shift, Monday-Friday, plus rotating weekend call: Friday night through Monday morning (issues are taken care of by phone unless it is an emergency).
