Job Details Job Location: Johnson Creek, WI 53094 Location: Hybrid; Driving to patient homes and facilities. Position Type: Full Time, Salaried. Schedule Requirements: Primarily day shift, but also includes required on-call, weekend, and holiday rotation. Purpose of the Position This position will be responsible for evaluating patients for potential admission to Rainbow Community Care and will assist patients and families in their adjustment to terminal illness and grief. You will help resolve personal, financial, and environmental difficulties that occur with impending death. May also provide grief counseling based on family need. Key Responsibilities Completes psycho-social assessments of patients and families and completes all other documentation that is necessary for the patient’s plan of care. Assists in the development of the plan of care with other members of the Interdisciplinary Team. Has knowledge of and keeps an up-to-date resource file of government programs, insurance data, and community resources available to enhance care of patients and families. Attends Interdisciplinary Team meetings as the Social Work member reviewing patients’ and family care and offering appropriate input on psycho-social issues. Delivers customary bereavement follow-up provided by our program. Provides education and/or support concerning Advance Directives and Advanced Care Planning. Provides guidance to referral sources and patients and families about the benefits of hospice services. Provides information to patients and referral sources about the intricacies of the Medicare Hospice Benefit. Develops relationships with physicians, hospitals, and assisted living facilities in designated territory. Personal Qualifications: Ability to maintain patient confidentiality. Must present self in a professional manner at all times. Inspires a culture of trust, acts with integrity, and demonstrates good work ethics including good attendance and punctuality. Ability to be self-directed and apply problem-solving techniques in resolving issues. Must be able to drive to and from patients’ homes and to meetings at other locations as necessary. Must be able to work reasonable amounts of overtime and be flexible with hours. Must be able to work on-call hours, weekends, and holidays as necessary. Must be able to maintain healthy boundaries and positive interpersonal relationships. Professional Qualifications: Active Wisconsin Social Work license is required and must be maintained. Valid Wisconsin driver’s license and current auto insurance required and must be maintained. Minimum of 3 years public health, home health or hospice experience preferred. Knowledge of hospice care, some medical terminology, and an understanding of emotions and feelings related to death and dying required.
